Here are the steps to request a new consult or follow-up appointments:

Go to the calendar below and select a service.

New Patient (15min consults) - Follow up -Existing clients (choose appropriate tab)

Choose a Date and time

Complete the questionnaire complete if you are a new client.

(New Patients who prefer a Intake 60min) Send a copy of your photo ID (State ID or Driver’s License) AND insurance card (front and back) to admin@idealmentalcare.com

Once we approve your request, you will receive an invite from the Charm Health Portal to complete the intake forms, questions, and consents.

You will also need to enter your credit card information under billing.

Finishing these items will ensure a smooth appointment.
