1. Open your State calendar and select a service, date, and time.
2. New Patient (15min consults)
3. Complete the questionnaire.
• you will receive an invite from the Charm Health Portal to complete the intake forms, questions, and consents.
• Finishing these items will ensure a smooth appointment.
